How to put in a service ticket?

I'm having issues with my app disconnecting from my POWERNODE even though the light on the unit show's it's connected to WiFi.  

It's happening often and the only solution I've found is cycling power to the POWERNODE.  

I'd like to put a service ticket in but the "Send Support Request" button on the app brings up a blank page.  

Please let me know the best way to start a service ticket so I can resolve this.
