Please explain how the NANO's 12v trigger works

Since the NANO is always on or on standby, is the trigger activated by input?  In other words, when there is no input to the NANO, the trigger is off; and conversely, when there is active input, the trigger is on?  

Second, is the 12v constant or a pulse?
