    Try this..

    Reboot your router
    Wait 5 minutes - very important step
    Reboot all your players
    Wait 5 more minutes
    Relaunch the App

    Another C658 owner here. Not having this issue with 4.0 on iOS or Mac App, all input sources are still visible from the Customize sources menu.

    Have you done both the app and firmware upgrade?

    Brian, Yes, both app and firnware was upgraded. Firmware version 4.0.2. Thanks
